The proposed $500 Deductible Plan would be modified and renamed the Latitude Health Plan, which will be designed to help members improve their own health and help us manage rising costs.
To be eligible for the Latitude Health Plan in FY 2013, you and your covered spouse must complete the Health Assessment between January 1, 2012 and March 31, 2012, or you will be limited to the two higher deductible health plans for FY 2013.To complete your Health Assessment, click here.
